When the alien in possession of his/her passport and a valid visa arrives and applies for admission to the United States, if admission is granted, the alien is issued an I-94 specifying the visa type, date issued and date the alien must leave by.

Everyone entering the US with a valid visa who is not a citizen or an LPR will have an I-94. Those entering under the Visa Waiver Program will have I-94W.
This is the record of the alien’s arrival and departure, along with the status they were admitted as, any additional endorsements (such as temp employment authorization) and the date their status expires (and that being the date they have to leave).
